"I have everything I'm supposed to have. So why do I feel so empty?"
Many people between their mid-forties and mid-fifties feel this way: the career is stable but you can see where it ends, the family is intact but the children no longer need you, your parents are starting to decline and your own body is not what it was. Nothing is seriously wrong, but every morning you wake up tired, thinking "is this it?", feeling you are on a track with no exits, and that the time left is shorter than you imagined.
Midlife transition is an increasingly common reason people come to counselling. This page explains what midlife transition is, why it can leave people feeling lost, how counselling can help, and when it may be time to reach out.
Midlife usually means the years between forty and sixty, the stage of life that carries the most roles at once: parent, child, partner, employee or manager, and sometimes caregiver. What most defines this stage is that much of what was built in youth begins to change at the same time: children grow up and leave, parents decline or die, careers hit a ceiling or face redundancy, the body begins to age, marriages settle into long-term weariness. And in the middle of all this, a person feels clearly for the first time that time is finite.
These changes are themselves normal, and everyone goes through them. Some people adjust gradually along the way, and even find a clearer direction in the second half than in the first. The trouble comes when these changes arrive together, and a person's identity and worth have rested on the very roles that are receding. Then they may fall into a nameless sense of being lost, empty or panicked, and begin to doubt the choices of the past few decades.
The pop-culture "midlife crisis" is the dramatic version: the sudden sports car, the divorce, the resignation. Research shows this version is actually uncommon. But research also finds that life satisfaction does tend to reach a low point in midlife, forming a "U-shaped curve": declining from the twenties and thirties, bottoming out in the mid-forties to early fifties, and rising again with age. This low point is not because middle-aged people are especially unfortunate, but because this stage carries the most responsibilities and the most losses at once, and the gap between what you expected of your life and what it is becomes clearest here.
Developmental psychology calls the central task of this stage "generativity versus stagnation": people begin to ask what, beyond their own achievements and duties, they are leaving for the next generation and the world, and whether their life has meaning. There are no standard answers, but if these questions are kept buried under busyness, they surface as emptiness, irritability or unexplained low mood.
Part of midlife transition is physical. Women usually enter menopause in their forties, and hormonal changes can bring hot flushes, insomnia, mood swings, and changes in memory and concentration. The Department of Health's Family Health Service has a fuller introduction on its menopause page. Men also experience a gradual hormonal decline, showing up as tiredness, low mood and reduced drive. These physical changes intertwine with the psychological ones, making it hard to tell whether a low mood is bodily, psychological, or both. The physical side can be handled by a doctor, but what this period means to you, and how it changes the way you see yourself, is something counselling can help you work through.
Feeling lost in midlife is rarely caused by one thing. It is many changes piling up in the same period, each manageable on its own, but together throwing a person off balance. These are the most common sources:

Midlife difficulties are more than "feeling down." They show up in emotions, the body, relationships and behaviour at the same time. Some common signs include:
In Hong Kong, people aged forty-five to sixty-five number more than 2.3 million, over thirty percent of the population, and a survey by the Golden Age Foundation and HKU found this generation in need of financial and mental health support, yet few seek help. Feeling lost in midlife is usually dismissed with "that is just what middle age is like" or "what have you got to be unhappy about, you have it all," and few see it as a period of transition deserving serious attention.
The caregiving burden of the "sandwich generation," inadequate retirement provision, financial worries driven by housing and living costs, the difficulty of finding work after a midlife layoff, and the recent wave of emigration that has split many families, with parents staying while children leave or people emigrating while parents remain, all make midlife in Hong Kong especially hard. When a person carries all of this while society expects them to "cope," many spend years quietly wearing down.
Counselling is not about getting you "back to before," and it is not about deciding for you whether to change careers or divorce. It is about sorting through what is happening in this stage together, and rediscovering, beyond the roles that are receding, who you are and how you want to spend the second half. Research shows that after life satisfaction bottoms out around fifty, it keeps rising, and many people are more content in their sixties and seventies than they were at forty. The midlife low is real, but it is a turning point, not the start of a downhill slope. Counselling helps you avoid getting stuck in the low, and find your own way up.
